It depends on the approach you want to take to the MR but if you are hoping to get the Decision revised then you need to deal with each activity that you did not score the points you expected for.
If there is reason you are having to buy the pads yourself, as your post suggests then mention the usage and explain why they are not prescribed.

hi again, i am reading through my assessment report form trying to find some points to use in my MR letter and i am confused with some words used in the justification for descriptor choice, can you please explain what the following means ie, the CQ, the FME, the HOC, the FH, the SOH, the IO, and the MSE, sorry if this question has been asked previously , many thanks confused bb.

CQ – Claimant Questionnaire
FE – Further Evidence
FH – Functional History
FME – Further Medical Evidence
FRR4 – Telephone contact form
HOC – History of Conditions
MSE – either Mental State Examination or MusculoSkeletal Examination depending on context
IO – Informal Observations
POI – Proof of Identity
SOH – Social & Occupational History
