Hi had been turned down on mandatory reconsideration for my dla to pip .I am going to take it to appeal I have contacted my neurologist Secretary and they are going to do a letter for me .Is there a template for gp or consultants as to guide for them on what to write i am due to see my consultant in the next few weeks and if i had some type of a form template it would be easier and quicker to process also any advice would be greatly appreciated .Thanks
