Your Guide to claiming PIP states “In relation to supervision, prompting and assistance DWP guidance explains that: ‘the assessment will take into account whether individuals have an underlying need for support from another person, regardless of whether they have access to this’’. So, if it would be reasonable for you to have support, even if you don’t actually get it, then you should be scored on the basis that you need it.”
When completing the PIP2 Form for my disabled son, where I have ticked a Descriptor indicating supervision, prompting or assistance is needed but he doesn’t actually get it should I explain in the Extra Information box that he doesn’t actually get it? Or not explain?

If he needs additional support but does not receive it then you need to explain how he manages on a day to basis. As an example; if you are saying that he needs prompting to prepare cook a meal, then how does he manage to feed himself without actually receiving the prompting.

Your Guide to claiming PIP states "Where your ability to carry out an activity is assessed, you are to be assessed as satisfying a descriptor set out in column 2 of the table in Part 2 or 3, as the case may be, of Schedule 1 only if you can do so –
(a) safely;
(b) to an acceptable standard;
(c) repeatedly; and
(d) in a reasonable time period."
Do you need to be unable to to do all four of these for me to able to say he is unable to reliably complete an activity or is it sufficient just to be unable to do one of these to say he is unable to reliably complete an Activity?

Only one needs to be met to be unable to complete an activity reliably, also activities must be completed on the majority of days, so consider whether this is applicable as well.
