Asking to hear what the accessor has written

There are a lot of horror stories out there about claimants receiving zero points in their PIP assessment only to discover that part of what the accessor wrote down bore little resemblance to what was said

That been the case is the claimant or their companion (I’ll be accompanying my GF who has bipolar and is really cracking up under the strain and will need someone with her) entitled to ask the accessor to read back what they’ve written if only for ‘clarification’ purpose.

If they are should this be after each question or at the end of the assessment
