Hi,

So I'm wanting to make a Subject Access Request with regards to the stopping of my PIP, because I canceled a 2nd assessment appointment. I do have this template for a SAR request:

This is a Subject Access Request.

Please make sure this gets to the correct department. I had a face to face assessment at ( office address) at (TIME) and require the following information

1. PIP2 Questionnaire
2. PIP Review File-note – PA1
3. PIP Consultation Report Form – PA4
4. PIP Payment Decision Letter
5. Statement Of Entitlement Letter
6. DPA – Computer Printouts Of Everything On Their System To Do With Your Claim


The problem is, that's more for if you've just been assessed. The information I'm more after, is after I filled in a PIP.1043/AR1, and the attempts by me to change my assessment to a home assessment with the help of a doctor's letter, and the cancellation of my benefit after I cancelled the 2nd appointment.

Is there any other documents I could be requesting? I'm also after logs of attempts to contact me by IAC and alleged text messages that never arrived, and the communication between IAC and the DWP with regards to this.

Forgive me but I'm unsure what you are trying to achieve with this request.

Some of the documents will not be available as you did not have an assessment, so there will be no PA4 for example.

Th einformation you are seeking is split between the DWP and IAS. So things that would be included in your file; your PIP2, will now be back with the DWP. Information about the assessment you were asked to attend will be with the IAS.

From your posts you appear to have been properly notified of two appointments and failed to attend. Any additional attempts to contact you by ISA may not be relevant to your appeal, it is the formal notification that matters.

I can't think of any additional documents that may be available, the only other thing you might request would be copies of any phone recordings that had been made, but again this needs to go to IAS not the DWP.

Well the letter I've received from the DWP makes out IAC made a mysterious extra effort to contact me via 'several text messages' that I did not receive, to rearrange the 2nd appointment, after I'd already been told when I phoned to cancel the 2nd appointment (and did), that I couldn't rearrange it, and it was now out of IAC's hands, and was passing back to the DWP. The whole thing seems a bit suspect, and against what I understand their standard procedure (i.e. what I was told on the phone).

No, I don't really know what their motive is for claiming this. Perhaps it's to try and help justify what I, at least, would regard as a harsh decision. Perhaps it's meant to be a distraction. Who knows, maybe they did break procedure to make an extra effort to sort things, but as these text messages never arrived, pardon me for being a little skeptical.

So yes, what I'd like is to see some call and text logs from IAC to see if these contact attempts actually happened and weren't invented. Is it possible to SAR IAC for these logs?

Write to IAS in the same format as you intend for the DWP. I'm unclear as to what information they record and also what is retained, but you have nothing to lose.

The law requires written notice of the assessment to have been sent no less than seven days before the appointment, any additional notifications are outside of the legal requirement.

Will do, Gordon, thanks. I think I'll just ask for every document they have on my claim to date, instead of a list.
