Afternnon all
Gordon need advice please on what i need to put in the grounds for appeal section 5 on SSCS1 form. I still havnt received the HP assessment report, despite three phone calls to DWP. I only have until Monday 12th March to lodge appeal.So need to get it sent recorded delivery today, or morning at latest. I cant argue any points until i see the aids the CM alloted for points gained and cant argue HP comments until i see them. Thanks in advance.

There is a sample completed SSCS1 in the Submission PDF in the PIP section.
You don't need to go into detail on the form, just sufficient for your appeal request to be accepted, you don't need sight of the report to do this, you can use the information from the Decision Letter.

Hi Gordon many thanks for your reply, im even more confused now, I went into the PIP Mandatory reconsideration and appeal link; and data on here has confused me even further. Firstly its title says *appealing against disability allowance or attendence allowance*; when selected.Then the info within it clearly states that i should have been contacted by CM, to discuss his assumptions on the HP report and his assumptions on my assessment. So i am beginning to think that i have to have another Mandatory reconsideration before i can appeal, as the first Mandatory reconsideration was purely based on that they considered that i didnt give a good enough reason for not attending an assessment (even though i had sent a special delivery letter 5 months earlier and never had a reply until they cancelled my DLA) and had previousley had home visits without requesting them, when needed.That was the reason they cancelled my DLA and for saying i was not entitled to PIP. They never stated they had looked at it! Just i wasnt entitled to it because i didnt attend the assessment. Which i wasnt physically and mentally able to at the time. So maybe i do need to ask for a second Mandatory reconsideration before i go to appeal.

As long as you have a Mandatory Reconsideration Notice in regard to your award of PIP, not the missed assessment, then you can go on to appeal. If you don't have an MR Notice following the PIP award then you need to request one first.
As to the guides, from your post you have followed the wrong link, everything you need is on the following

Hi Gordon can you send me a direct link to to document on this site that has example, as i cant find it on my laptop. i spent two hours yesterday trying to download the appeal form from your site and had to go to cab site in the end to download it. Many Thanks.
