Are you absolutely sure that you are only receiving ESA(CB)?
You can contact the DWP office dealing with your ESA claim and ask why you have been sent the request given you are only receiving ESA(CB), hopefully they will withdraw the requirement, but if they don't then you need to reply as failing to do so can result in your ESA being suspended.
