The ESA legislation does not provide for any extension of the four weeks you are given to complete and return the form, however, the DWP may accept the late return of the form given your circumstances.
So first of all contact the DWP to find out whether a late return will be accepted, you need to make sure that they contact Maximus about this.
If they won't then I would complete the form as you can in the time remaining and then send it as is and then follow up with a letter covering the missing information, this should be sent to the same address as the form, also send a copy to the DWP office dealing with your claim and if you have a face to face assessment take a copy with you and ask the assessor to read it before they continue with the interview.
