Claiming backdated ESA disability premium

Hello,
I'm new to this forum. I receive ESA contributory in the support group and DLA. I recently moved to another private tenancy, and when I phoned ESA to tell them my new address, they said that due to a court ruling, people who were moved from incapacity benefit to contributory ESA should have been assessed for an income related disability premium, but were not.
I have been sent form ESA3 to complete to assess my income.
My concern is that the form says not to complete if you're on contributory ESA and I don't want to be flagged up for another ESA assessment,
Is it correct that I should be filling in form ESA3 to receive backdated disability premium I never received?
Thanks

In case you are not aware, your real name appears to be showing in the forum, if you want to change this then follow the instructions in the following FAQ

My full name is showing, how can I stop it?

This is just a financial assessment to see whether you are eligible for an Income Related ESA top-up to your current Contribution Based ESA payments.
