just a short message just gor my reveiw form but it doesnt have tick boxes lots of questions should i put more information in and get some new letters from consultants and omy OT just received today and they say i have to return before 1st oct that doesnt give me a mont thanks for any advice.

The DWP appear to have updated the AR1 to a new format in the last month, the current PIP Claim guide has been updated to cover it.
