Hi, im trying to help my nephew complete his PIP review form and wondered if there is a sample that may help? He is an amputee with severe mental health issues and as with many others, this has triggered a MH crisis for him. He moved house to an adapted bungalow a few months ago and has "lost" his record & evidence of previous claims etc which is making this very difficult. He has been uncommunicative & didn't tell me he was being reviewed until after the return date - I've managed to get an extension but that is getting very close now. I'm not great with technology unfortunately so would be very grateful for any help you could offer. With thanks
