UC50 - "other Healthcare Professionals" was created by Steve Rogers
Hi, I'm Steve. I care for my disabled wife Emily.
Emily has temporal lobe epilepsy (complex partial "absence" seizures, aura, confusion and memory loss) for which she receives DLA - middle rate care, low rate mobility - and I receive Carer's Allowance.
We have just started Universal Credit. We have not applied for ESA before but appear to be eligible for its equivalent within UC. We have submitted our first Fit Note which states "not fit for work" and "will not need to assess your fitness for work again".
My question is this:
Emily was diagnosed many years ago with an incurable and untreatable disability, so we don't have much recent GP contact. I am definitely the person who knows her condition best as her husband and carer. Should I fill in the "others who know the most" section with my own name, with the name that of the neurologist who saw her over 10 year ago, or leave it blank?

My full name is showing, how can I stop it?
Put your wife's GP down as the contact, it needs to be a doctor.
If you have not already done, then you must request that your wife has a Work Capability Assessment, this should be done via her UC journal bit I would also contact her JC+ advisor.
If the Fit Note is an indefinite one then the DWP may not accept it they usually require Fit Notes with a period of no more than three months in the first six months of the claim.

Yes, the reason for the query is that we are in the process of filling in a Work Capability Assessment. I have filled in her GP surgery as main medical contact but was asking about the "other" question below it.
Yes, the Fit Note is for a period of three months, not indefinite.

You can put your information if you have no better contact but don't assume that they will contact you, any testimony you want to make needs to be attached to the UC50 when it is returned.
