Hi All
I'm already in receipt of the standard rate of the PIP for care (nothing for mobility).
I was awarded it for 5 years and 2 years into this I contacted the DWP re change of circumstances (significant deterioration greatly affecting my wellbeimg and day to day ability. This includes new diagnoses/medical evidence sinse then).
I am under 6 specialists and have numerous medical appointments.
I am currently on ong term sick from work.
DWP informed me I need to complete a how your disabilities affect you form and I received the same form to complete all the information like when I first made the application.
I have returned this (on 23/11/2018) with a lot of new evidence including reports/clinic letters from all involved in my care.
I have recieved an appointment today for a f2f assessment on 31st January.
I have since seen information re it should be a different/shorter form for a review of award.
I'm now worried it's the incorrect process and it may mess my claum up.
Could somebody kindly shed some light and advise accordingly.

No, you have received the correct form, as you have notified the DWP of a Change of Circumstances it is always the PIP2 that is sent.

My confusion was due to being informed as well as reading reading on here re the AR1 form forchange of circumstances.
In which circumstances is the AR1 sent for? Is this the form used for when the DWP review the award when the period of award is coming to an end?

I'm not sure where you have read this, a PIP2 is always used where the claimant reports a Change of Circumstances.
The AR1 is used when the DWP initiate a scheduled reassessment on an award of more than two years and some awards of two years.
