capabilkity for work questionairre recieved

hello,
i have reciieved a capability for work questionaire form today, it asks for supporting information, such as a dr's letter, i have one, but it is from november 2013, things have not changed a great deal since, if anything, my condition has worsened, and i have had to have four surgical procedures for a new problem, which is now hopefully sorted out, but i will need to been screened every year i believe to see if it has returned, what i want to know is it worth sending my dr's supporting letter, or is it too old to be relevant?
any advice in helping me filling the form out much appreciated,

Realistically the GP letter is too old, would they provide an updated one?

it says on the questionaire, only send us copies of medical or other informationif you already have them, don't ask or pay for new information or send us original documents. so where does this leave me? i feel a bit confused about what to do

Ignore it, the DWP have no right to tell a claimant what they can or cannot include with the form.

hello again,
i have been to see my dr today, and he has given me an updated letter of support and a current list of medication that i take, the thing i would like some advice on is, when i send the questionaire back, should i mention to them that they have no right to tell me what i can send back, which my dr confirmed, or should i just send the updated details without mentioning it, i do not want them to ignore the letter of support because they state i can't send new information, so what should i do? send it back and say nothing, or send it back and point out that they have to take the updated information into account?
advice would be greatly appreciated,

Just send the information and explain why it is relevant to your limitations in the form, they will look at anything that you attach.
