Hi I have been on the highest award for Care and mobility for DLA then 1 year ago I was assessed for PIP and Awarded the Highest rate in both categories. I was told I was being reassessed after 12 months as there was only 12 months left on the claim.
I filled in the claim form and went for the assessment. My condition hasn't changed I suffer from Chronic PTSD and Chronic Pain due to injuries I got through being attavcked on duty as a police officer.
I have an assistance dog and never leave the house without her or being supervised by someone else. I went out alone about 6 months ago, lost my memory of the prevcious 12 years and was found several miles from my home in Cheshire believing I was going to work in Lancashire Police about 70miles away.
I need to be reminded to take medication and I can only walk for approx 1 minute at a slow pace then I stop for a few seconds to rest. I walk my Daughter to school each day which is about 200m away and it takes me approximately 30minutes for the journey. I cant drive without being supervised and I am about to lose my licence due to my illness and the symptoms.
I requested a copy of the assessment report and prior to getting the decision I recieved it. It was full of lies. Some blatant other twisting the truth. I have made a list of what is wrong and it is a five page document which points out every lie. They say that they completed a mental state examination which I passed. I cant see how I can when I am shaking most of the time and cant leave my home due to dissassociation and memory loss.
I dont know what to do. The assessor has blatantly lied from simple things such as I dont shake to not mentioning my need for an assistance dog. The list is long.
I know I need to do a mandatory consideration but not sure what evidence I can put in as it is my word against the assessor. eg I shake, I can communicate etc.
Also I was given details to make a complaint against the assessor. Is this something I should do?

You have the report but have the DWP made a Decision yet?
If not then there is a narrow window for you to write to the Decision Maker at the office dealing with your claim detailing your concerns about the report. Try and stick to issues that lead to your scoring points and if you do comment on the assessor's opinions then try and do so by reference to your evidence.
If you have had a Decision, then the first stage to challenging it is for you to request a Mandatory Reconsideration, this needs to be done in writing to the DWP, within one month of the Decision, to the office that dealt with your claim, have a look at our PIP MR & Appeal guide for details of the process, the PIP area also has template letters that you can use to make the request with.

Your primary task is to show that you meet the criteria, there are many reasons you may have failed, you need to address each of these but don't get bogged down in criticising the assessment report unless you can clearly show that it is incorrect, it is a lot easier to argue the facts of the situation;
"the assessor recorded that I walked 50m, I did but they have failed to document that I had to stop every 10m for a rest due to breathlessness"
than their opinions
"based on my observations of the claimant walking I believe that they can reliably walk more than 200m.
When you have a better idea of the issues with your claim, come back to the forum and we will do our best to help.

Thank you I have sent off a Mandatory review request. I have seen my doctor and he said he comes across this a lot and he will support me a write a report if necessary. I thought one should be requested as when I filled in the form it said do not get any letters from your G.P. On the assessment it just said F.M.E. I haven't a clue what this included. I am worried as the assessor blatanty lied in the report and I have now lost a large proportion of my income. Since the assessment I have had my medication doubled and my driving licence will be revoked due to my illness.

I'm afraid any changes in your conditions since you were assessed cannot be considered in your MR or any appeal that you might make.
