Hello..
I have received esa50 reassessment form at weekend.. Without going into horrid detail i am in state of physical & mental collapse & trying desperately to keep going.. Gp has visited & given us letter basically stating that she is significantly worried about me (scary), that my conditions are severe & lifelong with no realustic prospect of recovery.. Only 18 months since last assessment..She has given some detail of my illnesses.. Told them my mp has been involved & that she has had discussions with tnem in the past & asked them to use previous assessments & exempt me from these repeated assessments..
I would like to ask you if you think tnis letter will help in any way & what addfesses i send it to.. She has asked me to send it straight away before i even manage to get started on the form..

Dealing with the letter first.
Send it to the DWP office dealing with your claim and to the same address as your ESA50 needs to be returned to unless you can get a better address by contacting Maximus.

It's very difficult for me to comment on whether the letter will allow a Paper Based Review to be done, the bottom line is that they have to be able to produce an assessment report for the DWP to make a Decision from., in part they will be looking at the risk to your health that the GP has documented, they will also be looking at your physical problems and how they fit with the ESA Descriptors.
You cannot ignore the form, failing to return it can lead to your claim being closed, so even if you put a minimum of information on the form, make sure it is returned to Maximus by the required date.

I am a bit confused tho.. i have the address to send back to same place as form will go ie. centre for disability assessments.. I do not know dwp address or which office deals with my claim or how to find it.. I am not great on computers because thought i posted this question last night.. Also what do you mean by better address?
