Hi
Could someone offer me a little advice on filling in ESA 50 form please?
I’ve been suffering from Anxiety depression/ panic attacks social anxiety for 15 years now!
I was moved to ESA in 2013/4 from Incapcity benefit, contribution based.

The last medical I had was 5 years ago! My benefit was stopped as the man doing the medical totally lied about what I’d said in the medical!
I appealed to a tribunal, I couldn’t attend due to severe anxiety( I don’t go out much, only very early in the morning when not many people are about)
To cut a long story short I won at tribunal and got my benefit back dated!
They moved me to support group and left me Laine until now.
The last 4 years on top of my mental health problems I keep slipping my disc which is excruciatingly painful
It’s actually happened now so I’m medicaticated and not quite with it!
Ive never claimed for the disc issue even though it’s really effected me as I can’t sit( tend to lay or slouch of sofa! Standing for long periods is so sore!
I’m just worried about the esa 50 form and could do with a little help please!

Thank you Gordon, I’ll have a read through your guides!
Problem I’ve got is because I hate waiting in waiting rooms like docs I tend to call me doctor! I even get the chemist to arrange my meds so I don’t have to go!

Just worried sick these assessors will try and scam me some how! Like they did to me previously!
