Is there any medical evidence from your DLA claim that youthink might help us understand how your disability affects you?NoYesGo topage 19. We will not use any medical evidence from your DLA claim. Please use the following medicalevidence (tick all that apply):This evidence could be a report from: a GP, hospital, school or otherhealth or social care professional. If you've had a medicalexamination for DLA we could use the report from the examiningmedical practitioner (EMP). If an Employment and Support Allowancereport was used to support your DLA claim we could use this report.GP reportHospital reportSchool reportEmployment and Support Allowance (ESA) reportExamining medical practitioner (EMP) reportConsultant report or supporting letterHealth and social care professional reportOther medical evidence. Please tell us what other medical evidenceI want you to use all the medical evidence from my DLA claim.I want to see all the medical evidence from my DLA claim before I decide. If requested, we can get your DLA file and make sure this medical evidence is used.Because of the laws about Data Protection and how long we're allowed to keepdocuments, old evidence may not be available. If evidence is still on your DLA file,we'll make sure that it is usedetc

The DWP is asking whether you want them to retrieve and use the evidence used for your last DLA assessment if it is still available.

If you opt for this then they are not going to send you a copy. If you want sight of it first then you can include the information yourself with your PIP2 if you have a copy. If you don't have the information then you may be able to retrieve it by using Subject Access Request.
