It's not usual but it can happen if the claimant is classed as being vulnerable.
You can still complete a UC50, you can download it from the following page
www.gov.uk/government/publications/uc50-...r-work-questionnaire Or you can write a letter, I would use the same format as the form.
TAke it with you to the assessment and ask the assessor to read it before they start the interview and to include in your file for return to the DWP. I would also send a copy to the DWP office dealing with your claim.
