ESA50 (05/18) Page 4 carers, friends or relatives? was created by Triumph62
Recently received ESA50 form for review of my claim, I am currently in Support Group. I notice the new form has different wording for the section for providing the name of someone who knows the most about my illness, that now includes friends and relatives. Is it really necessary to complete this section and the related Other information panel on page 21? I didn't find any mention of friends or relatives in the latest ESA50 guide for that section.
Due to my situation, living alone, with CFS/ME, I rarely see anyone as I rarely leave my home, and only speak to 3 people a week on the phone. I have no current treatment or support from the GP, the NHS or anyone else. I have previously not named anyone and just left my GP as the only contact, but I rarely see him, although I will before he completes the ESA113.

Nothing on this board constitutes legal advice - always consult a professional about specific problems

It is your form, you can complete it as you want so if you do not see a need to complete certain sections then don't

Hi Driver59
It's not compulsory to put down anyone's name except your GP - it's just there if you want to. Friend or relative contacts can be useful simply because they often have a better view of your day to day living than a medical professional - so you might want to consider with their permission putting down the name of one of the people you speak to regularly. However, if you don't wish to, it does not matter.
'The other information ' is to provide them with anything you wish to say, that did not go anywhere else on the form. Again - you are not obliged to fill this in.

BIS - thanks for your thoughts. I already have plenty of Other information to fill the page 21 section, so will do as I have done successfully before.
