My PIP was granted for three years and is due for reassessment January 2020 when do you think I will hear from them, what is normal

The DWP has been reassessing a year early but some members have reported being told that it will now be six months, it's unclear whether this change affects all claimants or only those who meet certain criteria or whether the decision is a regional one.
A new condition does not always mean a higher award, it is whether your limitations have also changed that is important and whether this would result in a higher award or not, have a look at the PIP Claim guide to see if you would score higher or not.

I have now also been diagnosed with cerebral small vessel disease by Amy neurologist and doctor after two MRI Brain scans. This has led to me being on long term sick and recently being retired for ill health. This disease has affected my day to day living and will eventually lead to Vascular Dementia. Should I let them know or wait six months. I have just applied for UC for the first time as I now have no income

Only you can make the decision about whether to report a Change to the DWP but I would advise you to see whether you would reasonably score the extra points for a higher award before you actually do anything.
