ESA 50 Form Missing was created by catvinnat
My sister has been seriously ill and I filled in her ESA 50 and returned it. We sent it via Royal Mail guaranteed service and I have the signed proof of delivery but they are saying its never been received . She is bedbound and we requested a home visit. They have sent her an appointment which I rang to change otherwise we would never have known that they 'didnt receive the form'
I now know we need to send a doctors letter requesting a home visit but I'm concerned about the missing form. Whats the best way to proceed to assist them in finding it?

Hi Catvinnat

You can try rining again and speaking to a different operator - saying that you have proof of postage, but if they insist they haven't got it, there is little that you can do apart from sending in a copy of your form and original evidence with a covering letter saying why it is a copy and attach a photocopy of your proof of postage and of signed delivery.

We had similar for a family member a couple of years ago at a PIP F2F, the F2F was a result of a review and on the form we had stated new issues. When the HCP came round (it was a home visit) she was about to end the assessment and I queried why she hadn't asked and questions regarding the new conditions, the HCP looked a bit shocked and miffed and asked "what new conditions I was referring to?", I stated "well they are all in the review form", she relied along the lines of "she had read the medical evidence I sent in but there wasn't a copy on the review form on the system, but it's fine I have seen your original claim form and medical evidence so I have everything I need".

I proceeded to tell her of the new conditions but it was clear she wasn't taking it in (I suspect she may have already closed the file on the computer by then) this despite us recording the interview (after a disagreement) .

We got a copy of the report and there was no mention whatsoever of the new conditions, turns out it wasn't important as the family member remained on enhanced rate daily living and mobility but as with you, had I not asked questions then we'd of had no idea the HCP didn't have access to the review form and was using outdated information.

Hi Catvinnat

If you sent the form in by recorded delivery, you can go on to the post office web site, punch in the recorded delivery number and you should be able to see who signed for the package, when and at what time, take a screen shot of this information and send it to DWP, you will still have to send a copy of the application form if DWP do not have it.
Whenever we send in forms to DWP we always take a photocopy of the form, if DWP do loose your form, which does happen, then you can take a copy of the copy, I always mark the photo copies as copy when I send them in.
