New Pip Form was created by Misterzero

I got my pip form sent out then got an extension for two weeks on it ,however a day ago my form got badly damaged ,will they send me out a brand new form

Hi Misterzero

We cannot speak on behalf of the DWP, but it seems a reasonable request. I would phone them straight away and explain what has happened.
