Hi. My bowel incontinence nurse has offered to do me a letter of support. Can you advise me how each component of how it affects me should be worded? I am permanently disabled due to this, which completely rules my life, along with constant nerve pain. It was in 1981 when I was incorrectly sewn after a 4th degree tear following childbirth. I suffered a life changing injury. I have had several operations, including 2 spinal implants with remotes, but no improvement. There is no further treatments available and this is permanent. Also is it worth listing, on my soon to be reassessed PIP form, 2 seperate lists of my illnesses. 1st list, permanent, chronic degenerating conditions, such as the above, M.E for over 30 years, Fibromyalgia, bulging discs in my spine, osteoarthritis etc to draw to their attention of giving me a light review instead of the 3 years I was given. As carpel tunnel, migraines, hip bursitis are conditions which are not necessarily going to get worse I’m not sure that’s why I was only given a 3 year award.

Hi Mememe

There is no special wording that your incontinence nurse needs to use. What you have written here is very clear and I would get her to write a version of what you have said here. The only things she needs to add is how often she works with you and confirm there is no more that can be done.
If you look at the guide GP notes - you could always print off the page about toilet needs and get her to tick and sign it and add it to the letter.

Regarding your medical conditions. I always write them in date order even though it is a very long list, but it's up to you if you want to have them as two separate lists.

You say you don't know why you got a such a short award and it can be a bit of a lottery, but if you still have your old claim, see what you wrote and where you may not have put enough emphasis on being able to repeat actions repeatedly and safely.
