Morning all,
On page 4 under The about other health professionals, carers etc, I’m assuming it’s ok if I put myself as the person who knows most about my husband’s condition?
I’ve created a separate list of the other health professionals involved.

Hi Magdelena
Yes, it's fine to put yourself. If you want to I would also write a supporting letter. People often think they don't carry any weight, but I believe they do. They don't have to be very long, but as you say, you know your husband better than any professional and are a witness to his daily struggles.

Much to my own surprise and delight, I have pretty much completed the form (with a lot of additional information) and now feel ready to craft a supporting letter of my own, as wife and carer.
There is a wealth of material, both functional and medical, in there already and I’m wondering how emotive a carers letter can be before in some way it becomes... counterproductive or exasperating.
His condition has devastated both our lives. It was young-onset and we lost everything, not only his health. I know no one can truly advise me on what to say, I guess my exhausted brain is worried I’ll sound melodramatic.
Thoughts?

I wouldn't worry about how you sound. You're the expert in this. Being with your husband every day means you see things that a consultant has never even seen and if you want to include a couple of sentences about the effect on you, you should do so. You've probably even seen things that your husband is not aware of because of the nature of his medical conditions. Just write from the heart and keeping in mind the criteria - write what you see, hear and experience of his daily life. Don't worry about trying to get it word perfect. You've done enough already!
