Home assessment ESA50 form 4 yrs old was created by jan777
Hi everyone,
I’ve been in the support group for over 4 yrs.
I went to the last assessment 4-5yrs ago due to physical issues, along with severe anxiety depression and panic disorder.
My assessment was cut short as I couldn’t breathe, vomited everywhere and the assessor who was a Doctor filled the rest of the assessment in so I didn’t have to go back.
I’ve received a letter for ESA assessment at home, I haven’t had any new ESA50 form so the last one I completed was approx 6 yrs ago ..
I’ve got a copy of it but it’s no longer relevant in many ways as I’ve adapted my levels of support as needed so much of the ESA50 form won’t correspond!
I could regurgitate the 6 yr old answers but it seems stupid as my health has deteriorated and now I’m more entitled if anything .
I was thinking of filling in a new ESA50 form and asking the assessor to take the new info into consideration as I haven’t got time to send it in due to I received the letter yesterday and the assessment is in 5 days.
Any advice and ideas would sincerely appreciated xxx

Hi Janet
It is never stupid to regurgitate 6yr old answers even if your condition has or has not changed, it gives you a starting point where you can add additional information as to how you are now.
I would make summary on how your condition has deteriorated and use the ESA50 form as a guide when compiling your summary and give it to the assessor when they arrive, also let them know the short notice you have had. If you have any support documents gather them together, remember to give the assessor copies of any support documents as you may not get the originals back.

Thanks so much for your ‘calm reply .
Your absolutely right Gary, this is part of my problem.. I can’t concentrate and then get really peed off with myself..
I had already thought of filling in a new form and also having my 6 yr old one to be able to answer the assessor properly rather than re think the assessment over and over again trying to remember what even happened!!!
I haven’t been out of my house since my Mums funeral in 2015 apart from attending A&E 4 times.
I had a home PIP assessment in June last year which is still waiting for a PIP tribunal date which I want to attend but probably won’t again.
At the home assessment I had a fractured tibia and fractured carpel something bone after my troublesome knees gave way and caused yet more injuries.
I had a full length brace on my leg and my hand was in a plaster splint and the assessor failed to mention it on her report and said I moved ‘swiftly’ across the living room (I was on crutches with a fractured wrist?
I’ve got an impacted wisdom tooth that’s turning around and cracked the next tooth, it’s causing me the worst pain and it’s so loose I want to pull it out myself.
I’ve been to my GP once in 6 yrs I have phone consultations if necessary.
I was on 2mg of diazepam and she refused to give the prescription unless I went to see her ... That was 4 months ago and I still can’t go ...She’s faxed 3 x 5 mg diazepam tablets 2 weeks ago to my chemist who’s brought them to mine as she thinks it’ll calm me to get the dentist and made me an hour appointment we’re only her and one nurse will be in the surgery to take blood tests , smear etc... I’d have to take all 3 and a bottle of red wine to even try and get there and still won’t be able to go.
SORRY... I’ve just realised I’ve gone off topic AGAIN.
So sorry,
Many thanks for your calming message xx
