Hi,
can you tell me where I can find the template letters for ESA extension of time to return ESA50 form due to mental health reason please. Also what are the significant reasons, if descriptor points are not awarded to stay in the support group ?

There is no extension available for returning the ESA50, you must either return it by the required date or late and if the latter then there is a good chance that your file will be returned to the DWP.

If you are struggling to return the form then our recommendation would be to return the form on time even if incomplete with a covering letter saying that you will provide the remaining information as soon as you can.

Are you talking about the Substantial Risk Regulations, they are covered in the ESA Claim guides?

Yes but also the exceptional circumstances rule.
Do you need to score 15 point in two of the questions to be placed into the support group ? I have also been on CBSG but I am unfamiliar with this new style ESA. What's the difference ?
Is it just how you are paid or is the whole assessment process different ?.
Very confused do to mental health worsening .

It's all in the ESA Clam guide.

You need to meet one or more of the SG Descriptors, Exceptional Circumstances or Exemptions.

From your posts your ESA claim goes back years so you are not on New Style ESA, you are on what is now called legacy ESA, just as you were before this Decision.
