You should contact the DWP for a copy of the assessment report if you have not already done so, I would phone them but again follow up the request in writing. Once you have the assessment report you will have a better understanding of how the DWP Decision Maker has come to their conclusions and will then be able to argue against them.
is there a template for this or cn I ask for it within the letter asking for an MR?
So it's:
Letter asking for MR, and Further time to complete add info, defer if possible due to my cancer, and copy of my assessment report?

To be clear you cannot defer the initial period, the one month is defined in law and cannot be changed so you would have to make a Late request citing your cancer treatment as reason, I don't see why this would not be accepted.
There is no template letter for requesting the assessment report, most members just phone the DWP office dealing with their claim to make the request, if you make the request in writing then it does not have to say much more that you want the assessment report {PA4).

No. As explained the one month (and when it starts) is defined in law and cannot be changed.
