HI
back here after 8 years, I only have to the 10th this week to make my telephone claim, but i would prefer to ask for a PIP 1, I have seen a copy of PIP1 and the detailed info required, surely the idea behind the telephone application is that they can say you are able to remember and communicate information , so will be noted on PIP2. Having said that I have cognitive issues and will struggle to answer and locate all the information they need. Can I ask if they include the 10th, if closing date for me is that day, tx.

Yes, the 10th is included if this is the date on the letter.
You have left this really late to request a PIP1! There is a real chance that your DLA payments will be suspended for failing to start your PIP claim by the required date.
Contact the number on the letter and ask for the form and an extension to start your claim.

I thought the main PIP form is sent to you after your first telephone call, do you mean If i don't get a PIP1 form in, then if thats the case then i will have to do the telephone call.

Your PIP claim cannot start until the DWP have a completed PIP1 so that they can generate a claim on the PIP computer system, the phone call allows the DWP staff to complete the PIP1 while you are answering their questions.
Once your claim is started then you will be sent a PIP2 to explain your health problems.

HI, compiling info for call now, in terms of agreeing to allow evidence from earlier DLA claims, ESA, EMP reports, is that a good thing, any downsides
would issues with memory, come under one of the questions they ask in the call, i have cognitive issues, brainfog, part of M.E, but it is not dementia,etc?

It's up to you, but be aware that the PIP assessment criteria is significantly different from the DLA one.
If you have had a recent DLA assessment or your conditions have not changed significantly since you were last assessed then you might want to consider using your DLA information.
If you have not been assessed for some time or your conditions have changed since you last were then you might want to concentrate on using new evidence.
Using your previous DLA evidence does not preclude you from supplying new evidence.
There is no guarantee that your DLA information will still be available.
Although you can ask as part of you making a claim, I would not expect the DWP to provide copies of the DLA evidence to you.
Our recommendation is to supply all of the evidence you want those assessing you to review with the PIP2. You can get copies of your previous DLA information, if you do not have copies yourself by making a Subject Access Request.
