Your guide on Capability for Work questionnaire says that the declaration on page 22 can be signed by a recognised Appointee. The wording on the form suggests it must be signed by the applicant. Could you please clarify the situation?
An appointee is a legal role made by the DWP for vulnerable claimants unable to manage their own affairs, where there is an appointee they are responsible for completing any claim forms and would be the person who would sign them.
Where there is not an appointee, it will depend on who completes the form and in what circumstances; so if someone completes the form on behalf of the claimant but using their words then only the claimant need sign, if the person uses their words then they should both sign the form.
In both cases, they should explain on the form what they have done and why.
