yes, refer back to the form but double-check the language you used on the form. is it possible to misunderstand the intent?

i dont think it can be misunderstood on the form i also stated this at the assessment

You say that you are happy with the language used, it's not impossible that the DM has not read that part of the form and is simply re-playing what is in the assessment report.
I would simply re-state what's in the form, I cannot follow a route and this is why.

The paper tissue is a common ploy. I was expected to take from the box when I was distressed, I thought it was odd and gestured to my husband to help. Having failed to manage this alone, the HP asked me to fold the tissue instead, indicative of what I don’t know. However having a sodden tissue in my hand crumpled as they do when wet, was used as a sign of strength to say I had no problems when I read her report later. How sad that a nurse indulges in petty games.
