Pip assessment query

Hi
I have a telephone assessment in a few weeks time and I know they ask about employment etc. However, last year I lived away from home for a few months for a new job, unfortunately I had to leave the job as my condition got worse which affected my mobility to get around and to do the job. I moved back home at the start of this year. However, I do worry that the form will be invalid, if they knew I lived away from home I won’t pass the assessment. Could anyone give any guidance on this? Could I just say that I had a job which I had to leave without saying where it was .
Also,is there any guidance on the phone assessment?
