My full name is showing, how can I stop it?
You can get a copy of your telephone assessment report by ringing the DWP. The number is on your paperwork. It's called a PA4 and it should be available 48 hours after your assessment (although some are taking a bit longer). If you are told that you cannot get your assessment report until after the decision - try and say that's not accurate, but if they won't budge - just ring again and try a different call handler. You are allowed to see your report.
