Uc50 form and new esa form was created by Rocker
Hi just like some information please I had bad accident December 2019 I have also other disabilities and have A indefinite sick so when I come out of hospital February 2020 I claimed UC and also a lady came out and assessed me and I filled in the UC50 form then and sent it in now other 12 months of ringing them they have lost it so do I have to fill it in again and will I able claim the new ESA i have worked till the accident now struggling am 60years old and live on my own all this forms are very confusing thanks

You are having a bit of bad luck.
A number of questions;
1) did you make a copy of the UC50 form
2) did you return the form by recorded delivery
You need to contact DWP and ask them, usually they will ask you to send in a copy of the completed form if you made a copy if not then you may have to complete a new UC50 form. check out our guide to help you. Our UC and ESA claim guide explains the criteria that you will need to meet.

New Style ESA is a contributory benefit. Normally, this means you may be able to get it if you’ve paid or been credited with enough National Insurance contributions in the 2 full tax years before the year you’re claiming in.
have you applied for PIP? www.gov.uk/pip if you are not sure if you are eligible try out our PIP self - test PIP self test

Hi yes I did send the UC50 from in by registered post and I live in one bedroom bungalow which I rent from housing association which lady works there who is welfare officer Helped me fill it in also she was with me November last year when got a call from Universal credit saying they where still looking at it and now seem as they cannot find it and just keep saying they are looking into it thanks
