This is a standard reminder letter sent out at the end of week three of the four weeks normally allowed to complete the form, it won't reflect any extension to the four weeks that might have been agreed by the DWP.
Have you returned your form by the agreed date?
If you have, have you checked with the DWP that it has been received?
