If you've sent your form off to the DWP, they should've scanned it and have it in the system by now. If you call the DWP helpline and explain exactly what you've said on here, they should be able to add it to the notes on the computer. Expect a long wait however. I've twice called in past fortnight and waited 48 mins and 54 minutes without an answer. When I did speak to someone, they told me that they were inundated with claims.
