I've actually seen this on my medical records before - it is only 1 page that's double sided. Can't remember the exact wording as such, but as far as I remember, it's them asking for to tick boxes on what the patient suffers with and lines to go into further details. However, I don't think some GP's fill these in correctly. I had mine filled in with nothing reported and no symptoms, despite having medical proof (from the same GP) of the symptoms prior to the DWP checkup.
