Yes I would reference any supporting documents as you complete the form.
You can reference the same document more than once through out the document. I always quote the document, the page number and the paragraph that I am referring to.
You can also write a supporting document as to what support you give to your daughter and why, the why part is the important part. Quite often I come across a supporting letter stating what support they give to a claimant but it does not state why they give the support.
