ESA to UC form, treatments was created by AC

Thank you for your previous help.

When the form asks for medical treatments does this mean to list all prescribed medications I take or use. I use a CPAP machine, would this be a treatment?

Or is this more for things like physiotherapy, B12 injections.

Hi AC

For the UC application form there is no need to list medical treatments if you are in the ESA Support Group.
