Hi Info
You can tick yes to the aids and still put an explanation that you need help to use them.
It's up to you how you choose to complete the form. Some people simply write on each question - "See additional page X and others write a couple of sentences and then see " See additional page ..."
Do what fits best for you. As long as everything is marked clearly, it will be fine.

Hi info
When conditions overlap each other, I always use approximately as quite often you get a condition before you become aware of a condition even though you may have symptoms, you are not sure what they relate too.
Quite often claimants are unsure what date to put, date they become aware of the condition or the date they get diagnosed. I always put the date they become aware of the condition as quite often it can take many months before they get an appointment to see a specialist.

Hi Info
Put what feels right for you. There isn't a 'correct' way to do it. It just needs to be clear what you're saying.
