Hi, sorry to highjack your post, but what is a PA4 please? Is it a copy of the assessment report, or the transcript of the call itself?
We went through a video assessment a few months back, me on one screen, my daughter on another. We asked for a recording of the assessment but assessor told us not possible, however, she could send us the transcript if that was ok. We said yes please, definitely.
Nothing arrived, decision came through, have had to do yet another MR, queried the Health Assessment and told by PIP to contact HAAS direct if we disagreed with their report. Done that, they say they have no audio or video of the assessment, and no transcript either. I asked HAAS if PIP have the transcript and they said no, just the report. Is that normal? Thanks for your help

I have moved your post. We ask people not to ask questions on other people's post because it can become confusing if we're trying to respond to more than one person in a single thred.
A PA4 is the name of the assessment report and it is generally available to claimants 48 hours after a telephone/video or face-to-face assessment. The PA4 is written by the health professional (assessor) and gives the claimant more idea of what the final decision is likely to be. Claimants can ring the DWP and ask for a copy of the report 48 hours after the assessment has taken place. If you haven't already got a copy of this report, you should ring the DWP and ask for it, even though I know you have already submitted your Mandatory Reconsideration.
With regard to a recording or transcript of your daughter's assessment. Not many people have had video assessments, so I was unaware of what they would make available if you had it recorded. Frankly, I would have expected an audio recording (but I can't find anything on their site) - so I have no idea whether you should have received an audio recording or a transcript.
What does't surprise me is that HAAS are now saying there is no recording or transcript. This frequently happens since the assessment services became responsible for providing recordings, hence why we always advise people to do their own recordings even if one has been organised. The problem with no recordings is that it becomes a case of 'assessor said, claimant said', and the DWP will always come down on the side of the assessor because there is no proof. The only slight advantage you have is that there were two of you and only one assessor, but it is still an uphill battle.
