Pip review

Hi,
Well the dreaded brown envelope dropped on my floor this morning for my pip review.
My day was ruined, I haven't eaten my stomach is in turmoil I took one look at the form and cried even to turn to my husband and say I can't do this again.
What I want to know is how the questions are set out on the AR1 form can I not fill them in and just say see attached and just write each question in my language because I'm having trouble trying to find the right thing to write in the box. I can do better if I write everything in one piece if that makes sense
