New style ECA

I filled in the online form for ESA and have a New Style ESA telephone appointment at 9.30AM tomorrow morning. the text says it's to enable me to agree to commitments in order to receive the new style ESA. I don't know what to expect from it - does anyone know what it entails? I do a little work for myself as and when I'm able to because I have fibromyalgia and other health conditions. I've got to fill in a permitted work form as well which I've received but not sure what to put for 'On what date did you start work or will start work?' (As I said I work for myself as and when I can so it's a case I've progressively been able to do less and less, it's not a new job or anything). And another question 'when will you get your first payment?' Again, what would I put there as it's not a new job? Many thanks in advance for any help you can give.
