Lost my PIP2 Form was created by Delphine

Hello
I have lost my copy of my PIP 2 form from 2022.
I have a review coming up so wanted to use it again as a starting point.
Has anyone previously been sent a copy of it from the DWP when asking them to send one out?

Hi Dephine

You could the PIP telephony agent and ask which docs they have retained. You are entitled to copies under a SAR ( subject access request ).
Alternatively you could request a copy of the report from the last assessment known as the PA4 which will provide a lot of relevant information.
