Dear friends,

I was given a 2 week fit note last Monday (03/09/12) which I sent in with my ESA1 form. My doctor this morning has now given me a 4 week fit note.

I have not herd back yet since I sent in my ESA1, the received it on Wednesday last week ( I sent in recorded).

Should I keep my new 4 week Fit note until I hear back or send it in now?

Hi Cr3,

I would send it in now, keeping copy.

This will ensure that your ESA claim is continuous.

It is always advisable to submit "fit notes" to the DWP ASAP after your G.P. issues them, even if there is an overlap on the dates.

Do I send it to the same office I sent my ESA1 form? Also do I need to add anything to the fit note like a letter?

Or just fill it in and stick it in and envolpe and post it?

Yes, send the fit note to the DWP office dealing with your claim. It doesn't hurt to include a short note stating what this is for - on't forget to put your NINO on any communication you send in.
