Your GP letter sounds very comprehensive, I think you should send it as is.
Recording is allowed even for home assessments. You must request it and pay the cost. The DWP will arrange it. Or you could take Gordons advice.

Neither Assessment Provider for PIP offer recording facilities for PIP, either at their office or at the claimants home, although you would still need their permission to make a formal recording.

But don't they have to make the arrangement?
I know it is for a professional recording provider to provide the facility.

Your link is for the WCA and ESA, where the Assessment Provider can (but is not required to), offer recording of an assessment.

Aha! Brilliant! Will send it tomorrow, hopefully they'll follow the GP's advice and offer a home assessment...
Oh I see re the recording.. So after I get the appointment confirmation I need to contact Atos or Capita to request authorisation for a recording?!
