Got the Date for PIP Medical and am Terrified

You can ask for a summary or a full print out of your GP records, I have both and my GP gave them free, including specialists letters they had received that I had not already had copies of.
I have just requested my dad's (different surgery)and they happily obliged but wanted the request in writing, signed by him, they also called him to ensure he was expecting the records and they dropped them off at his house. So far not one single medical person i have spoken to has stuck to 'they will contact us'. They all understand that does not happen. I paid for a specialist letter for my war pension deterioration claim from my GP, it cost £35 but was worth it as a copy for other things. Oddly enough the war pension people have actually contacted the hospital and my GP, they also did a home assessment with an actual doctor.

There is a guide for doctors completing forms, they are not allowed to give opinion so a letter giving opinion would be discounted anyway.

Ring reception, ask for your records, what process to request etc.

I know of the guidance that you speak of, but most comments or diagnoses are opinions based on facts and tests.

I have obtained many letters of support for people and a lot of them state "in my opinion", and most of the time the letters have helped to achieve an award.

I think the guidance that you speak of is to deter the Doctor from "guessing" and making such comments as "MR xx tells me that xxxxxxxxxxxxxxxx"

Even the AP Assessor's recommendations are based on opinions with respect to the medical evidence that they have and any tests and observations made.

Neither The DWP nor The AP can totally discount any evidence just because they see fit.

All evidence should be taken into account and the DM will eventually make a decision on "the balance of probabilities" taking into account all the evidence before them.

You can just put in copies of the letters the consultant sends you and the GP after your consultations. This gives a record of your conditions and how they change or stay the same over time. I've got a care plan which is also useful to include and the consultants also refer to that. i feel it all builds up a picture of your condition and the support you need.

It's my assessment tomorrow, really dreading it, had sleepless nights and my panic attacks are getting worse just thinking about tomorrow

Do they have a copy of PIP form that I filled out?

Do I take copies of all the information I sent to DWP with my form to the assessment?

I have read your guides and so has my husband, I just feel as if I am second guessing myself all the time and that I'm not going to be able to get through the assessment and then it will be held against me.

My appointment is at 1.15pm, I have read everything through again and all the advice on here and so has my husband. I have all the information they want me to take with me and I guess I can do no more.
