good afternoon Gordon, I just received my pip2 claim form today and it was sent 30 april and just got it today and they want it by 30 may, so I have under 3 weeks to do this as expected, may I ask as I am not sure if I fill in the descriptors pages I downloaded and printed from this site and give it to the gp to fill in comments, as in the booklet the same things are being asked but in a different way but the answers are the same which I will write in the answers, so if I give the descriptors to the gp for comments do I send that with the booklet ( as I am not sure if they would write and ask where I got a copy of the descriptors from ) or wait if they need more information from me, as I am asking my gp and specialist for supporting letters about my condition and how they affect me in daily and mobility needs, . this claim form is very daunting to fill in and has really stressed me already. also can my wife ( who is my carer ) fill it in for me as I shake a lot and at the end I just sign and date it ?. not looking forward to this journey at all. I am just very unsure about how I give the descriptors to the gp for comment. I look forward to your reply Gordon and hopefully you can help ease my stress and worry with your reply. lastly today are the descriptors you put here to download are also the same on the dwp website ?

If you mean the GP Notes document then you give this to your GP to either complete or to use to help them write their own letter, assuming that they are willing to do either. You would then attach the completed form or letter to your PIP2 when you return it.

The information in the GP Notes, PIP Claim guide and other documents on the site are all up to date and derived from the PIP legislation and DWP guidance.

Yes, your wife can fill it in for you, she should use "I" if she is using your words or "he" if she is using hers.

Make sure you both use our PIP Claim guide to understand what is actually being asked and the criteria you will need to show that you meet.

thank you Gordon, yes I was talking about the part that says dear gp, as the descriptors in that one has a yes no answer, but the one in my pack for me to fill in says for answers. yes no sometimes, and no individual box to tick for the one I want to answer, like cannot, or needs assistance, its a list of same basic things but no box so it would look like I an ticking yes to something I cant do ,if you know what I mean,sorry I am not explaining it to well to you

We have deliberately kept it simple to encourage GPs to complete the pages, you can cover the differences when you complete the form.

good evening Gordon and everyone else on the form, Gordon can I just ask you if I make a mistake in filling in my form can I tippex out the wrong answer I ticked and then put a tick in the correct answer I should have given? it was a misleading and trick question regarding mobility, I ticked yes I used a wheelchair but then I noticed it actually said "do you use a wheelchair to get around safely,reliabily and do this in a reasonable time repeatedly" I should have ticked no but as I do use a wheelchair even though my wife controls pushing it and getting it in and out of the car, I thought I had to say yes I use a wheelchair or they would say I was lying to them if I did not say yes, can I tick yes and no and explain to them why or should I just tippex the yes out. thank you. I am now nearly finished getting all my paperwork to send away to them ,so just want it over now

It perfectly OK if you do not think that a tick box allows you to explain your problems in a sensible to mark the box with "see below" or "see attached" and answer the question in the comments box or an attached page.

Tippex is fine.
