Now I cannot find my PIP review form, have been searching for ages plus I need an extension.
Should I phone up and ask for new form and extension at same time.
I am in panic mode and cant think straight, cant even think what phone number I need

I have found the form, but I definitely need an extension, can I ask for a four week extension over the phone to the DWP

So extensions only come in two week periods?
My form is due back mid Aug, so should I ask for the first 2 week extension now over the phone , and when that's granted immediately go on to ask for the next 'only DM to allow' 2 week extension?

Plus do I have to speak to a DM directly for the second two week extension please?

I have to make a claim for housing beneft at a new address in Sep. My understanding is that if I am still in receipt of PIP care then I will stay on my ESA rather than have to go onto UC. If I lose my PIP award due to this current reassessment and I lose it before I make the new housing benefit claim, will I have to go on to UC ?
