Will the assessor or decision maker have on hand
previous ESA forms that I filled out for my husband about 3 years ago?

Hi Radhika

The assessor is interested in the here and now so they will be looking at the current form not past ones. Whether they have access to them - I do not know. Obviously, the DWP has access to them.

On page 19 of the ESA form(Face to face assessment) can I request that my husband be assessed by someone who is qualified in the effects of a brain injury. I did request that for his PIP assessment, if he was going to have a face to face. However, for PIP, a paper based review was done.

This is because most of his problems are complex and related to his head injury and I feel that someone who is qualified would be better placed to assess him, For example, the GP had referred him to a neurosurgeon and not the practice nurse!!!!!!

Lastly, going through his old ESA paperwork I found the ESA medical report form. On there it states my husband meets the S/G criteria, as he has severe functional disability. For the Prognosis, it states that a return to work is unlikely in the longer term. How long would longer term mean?

Hi Radhika

Yes, you can make a request for a specialist and depending on the type of brain injury they should honour it. Let's hope they go for a paper-based review instead.

I can't tell you the length of time they mean for 'longer term', but it isn't any time soon.

I have sent off ESA by the 2nd of September deadline. I posted the GPs report on the 9th of September Special next day recorded delivery.
My question is that should I send a copy of everything, ie, ESA form , GP report etc to the Decision Maker? Or should I assume that everything will be sent to the DM.
